Already have a support passport? You can now add it to MIPACE

Choose between completing your Condition Support Passport in MIPACE or uploading your own PDF.

Your existing information should still be useful

Some people have already taken the time to complete a condition-specific support passport with their family, clinician, charity or support team. They should not have to type everything again just to make it available through their MIPACE profile.

Two ways to add a Passport

Within Condition Support Passport, choose Fill in my passport to enter relevant details directly, or Upload my own PDF to add an existing document. A parent or authorised guardian can make this choice for a child profile.

Decide what to share

Uploading a PDF does not automatically make it public. The account holder must enable the Passport and the Show on public profile setting. If they do, the profile displays a Condition Support Passport label and an accordion with an Open passport viewer.

The PDF stays outside the WordPress Media Library and is served through temporary, controlled access. Someone permitted to view it may still take screenshots or copies, so check every page before sharing.

Use a document you already have

The upload option can be used for a personally completed PDF support document from a charity, care team or another source. The Condition Support Passport is not tied to any one organisation or diagnosis. A document may contain sensitive details, so review every page before choosing to make it visible.
