Condition Support Passport

A practical way to explain your condition and what helps you, especially when you cannot easily explain it yourself.

What can I include?

Describe your usual symptoms and how they affect you.

Record what is normal for you, including observations if helpful.

Explain triggers, mobility needs and practical ways someone can support you.

Include relevant medicines and additional conditions only if you choose.

Choose whether the section is visible when someone opens your MIPACE profile.

Who is it for?

For anyone who wants to share condition-specific support information. You can set it up for your own profile or for a child you are authorised to manage.

The Passport is available for different conditions and individual support needs. It is not tied to a particular diagnosis or organisation.

How do I use it?

Open your MIPACE dashboard, enable Condition Support Passport for the profile that needs it, then edit the profile.

Add your own information, choose Show on public profile if you want it to appear, and save your profile. Turning it off later hides the section without deleting its content.

Privacy and important information

The Passport is optional and private unless you choose to show it. If you make it public, someone with access to your device link can read that section. Do not include unnecessary identifiers or information you would not want a stranger to see.

MIPACE does not diagnose, monitor symptoms, or provide emergency treatment. This information comes from the profile owner and does not replace professional medical advice or an emergency care plan.

Designed around the individual

MIPACE can display information entered into a profile or a completed personal PDF support document.

The person managing the profile chooses what is available. MIPACE is not a diagnosis or medical advice service.
